3: Commercial Documents (Agreements):
- The agreement must be signed and notarized in front of a notary public and include the following statement on the same page signed by the applicant and the two witnesses: “Before me appeared personally Mr/Mrs/Ms…….. and witnesses Mr/Mrs/Ms……… and signed this document, this day …/…./….”.
- The document must be authenticated by the Secretary of State. (please include link).
- A stamped, self-addressed return envelope.
- A copy of all original documents.
